Our AVICENNACARE primary care doctors and physicians take an approach to medicine where we don't treat just the illness, but the whole person. We love what we do and we do patient care. You do not have to be part of a family to come to our practice. We love to see individuals as well. Immigration Physical Exam

Dr. Haque is an Official Provider of the Immigration Physical Exam by USCIS for last 35 years.

Dr. Haque's office provide all your Immigration Medical needs whether it is immigration vaccinations, blood tests like Antibody Titers, IGRA Quantiferon Gold Test for TB or Chest X-Rays.

The following documents should be brought with you to your visit: Personal identification, your medical and vaccination records (if you have) and Form I-693. Medical examination is mandatory for all refugees coming to the U.S. and all applicants outside the U.S. applying for an immigrant visa. Aliens in the United States who apply for adjustment of their immigration status to that of permanent resident are also required to be medically examined.

The purpose of the medical examination is to identify any Communicable Diseases of Public Health Significance.
